Lunch Account Permission Form

As we enter our second school year using the cafeteria computer purchasing system, we have decided to give parents/guardians the option of controlling what their children are allowed to purchase using the funds in their lunch account.

To begin the year, all students at SCS will be allowed to use their cafeteria lunch account to purchase lunch, milk, ice cream and snacks. If you would like to block your child(ren) from using their account to purchase any of the items listed, please check the corresponding boxes below. Once you have completed the other information below, return this form to the main office at school. Please allow one to two days for the office to adjust your child’s account.

PLEASE NOTE: Once a child’s account is blocked from purchasing an item, they can still use cash to make their purchase.
